BOOKING TERMS AND CONDITIONS

Please ensure horses are in from pasture ready for their appointment in a CLEAN, DRY state. Damp coats result in a suboptimal assessment through palpation and may affect the effectiveness of the treatment. If the horse is not ready for the allotted treatment time it may result in a much shorter assessment and treatment time.

I reserve the right to withhold treatment if the animal is unwell, has an undiagnosed condition, or is considered dangerous. In such instances the payment terms below still stand.

Fine form equine Physiotherapy understands that there may be extenuating circumstances forcing the client to cancel their appointment. A minimum of 24 hours notice is required for cancellations. Full payment is due if less than 24 hours notice is given or if I attend a booked appointment where the client and horse are not present.

It is the owner/clients responsibility to notify myself if the veterinary surgeon has advised that treatment should be stopped or suspended for any reason

PAYMENT TERMS

Payment is required by cash or BACS at the time of treatment. Card payments and cheques are not currently accepted.

Fine form equine Physiotherapy reserves the right to pursue any unpaid accounts in any manner that is deemed appropriate. This may include the use of debt collection services or civil proceedings. Any costs incurred in recovering unpaid accounts will be passed onto the respective client.

Failure to comply with the payment terms will result in the withdrawal of the service.

For insurance claim cases Fine form equine Physiotherapy respectfully reminds the owner/client that the payment terms still apply and that the contract with the insurance company is between the client and the insurance company only. Therefore payment is required at the time of treatment. An invoice will be provided for the client to forward to their insurers to potentially re claim costs.

Pre payment is required for any new appointments or for anyone with prior debt history
